So, we are in a paradoxical situation, when we know a lot about genetics, pathogenic mechanisms and the role of enteric microbiota, have identified numerous risk factors, which with different extent of certainty may reflect a potential for development of IBD, but at the same time, no mechanism has been suggested as the leading or primary one, and this leads to a lack of specific and adequate diagnostic tests and procedures to establish the diagnosis of IBD.
